ESR ( Erythrocytes Sedimentation Rate )

Sample
The whole blood sample is taken in the EDTA ( anticoagulant ).It measure the rate of sedimentation in one hour.
It is stable for 2 hours at 25C and 12 hours at 4C.
Important Facts
•The ESR correlates with the plasma fibrinogin level and depends on rouleux formation of RBC.Poikilocytosis therefore tends to inhibit sedimentation.While increased flattening of RBC , as in obstructive liver disease leads to acclerate sedimantation.
•ESR should not be used to screen asymtomatic patient for disease,
•When ESR is raised the careful history and clinical examination will usually diagnose the disease.
Purpose
1.This is nonspecific test for diagnosis of chronic diseases like Tuberculosis and also raised in malignancies and autoimmune diseases.
2.Whenever any patient has raised ESR ,he/she needs thorough work-up.
3.ESR is raised with aging and anemia.
4.Patients with >100 mm /hour ESR may have malignancy, Rheumatoid Arthritis, Renal disease, Sarcoidosis,or infections.
5.Paraproteinemia e.g.Multiple myeloma,macrogobulinemia.
6.This test is useful for the diagnosis and monitoring of temporal arteritis and polymyalgia rheumatica.
There are conditions where ESR is not raised
1.polycythemia.
2.sickle cell anaemia
3.Spherocytosis.
4.Hypofibrinoginemia.
